How to File a Claim for my HRA or MERP benefit?

  1. Print the claim form for Hirsch Financial Services for your company intranet.
  2. Complete form and attach your Explanation of Benefits from BCNEPA.
    REMINDER: Only deductible expenses are reimbursable.
  3. Fax or mail form to the Hirsch number or address listed on bottom of form
  4. Please keep copies for your records
 

HFS Claims Processing Dates/Posting Dates

HFS has scheduled a check run for the following dates during 2010.  Checks should arrive approximately 6 to 10 days after each check run if your forms were received a few days prior to the check run.  These dates help you estimate when you should receive your reimbursement.

How to view your account with Hirsch Financial Services (HFS):

Go to www.hfsbenefits.com
  • Participant Login
  • Temporary User ID (SS#)
  • Temporary Passcode (1806)

Then it will ask you to create your own passcode.

If this does not work, call HFS at 410-771-1331 or 888-460-8005 for support.  If you made several attempts to login, you will need a new pass code from customer service to view your account.

If you are using direct deposit, you will receive a non-negotiable statement in the mail that shows the amount of the reimbursement and the date of the deposit.

What is the easiest way to find a doctor or facility that accepts our insurance outside the North-East PA area?

Visit the BlueCross BlueShield website at www.bcbs.com, go to the Health Care Coverage menu, select Find Services, select BlueCard Doctor and Hospital Finder.

Prescription Benefits

The “new” prescription benefits are now 3 tiers (per new contract negotiations).
  • Retail: $10 generic, $20 brand, and $35 non-preferred brand.
  • Mail Order: $20 generic, $40 brand, and $105 non-preferred brand.

This information is listed in the employee communication booklet and presentation available on the KCSD website under Human Resources (this page).

FYI – Formularies do change frequently. Your best resource is the BCNEPA website and select formulary. You may research any drug and find out what tier it is covered under. If you are currently taking a brand name drug and a generic is available ask your physician if you are able to take the generic form. This will be a savings to you!

There are times when drugs change tiers within the formulary. Unfortunately this is out of our control and is regulated by the FDA. If you have had this happen to you please ask your physician if there is a similar drug you can take that might be on a tier that is not as expensive.


 

Why is my provider making me pay deductible costs upfront?

Providers are not supposed to be doing this per BCNEPA. Participating providers are able to do this and it is becoming very common. Providers call your insurance carrier to verify benefits, provide a procedure code and they are provided with the eligible amount they will be paid and the amount that will be applied towards your deductible. You can contact BCNEPA customer service and make them aware if this has happened to you and they will contact the provider to see why.

School Security / Attendance (2 positions) (posted: February 8, 2010)

The Keystone Central School District is seeking applicants for security/attendance officer at our Bucktail Campus and an officer for district elementary schools.  Responsible for ensuring the safety of students by supporting laws, regulations and procedures. Must be an effective communicator and conduct oneself in a courteous and respectful manner; ability to work with diverse populations and experience working with young children and teens, desired. School security/attendance officer pay rate is $10.00 per hour. This is a school term position.

Minimum requirement is high school diploma or GED, valid Pennsylvania driver’s license with safe driving record, willingness to work flexible hours and work cooperatively with others. Security or law enforcement experience, preferred.

Athletic Department Positions Available for 2009-2010 (posted: August 7, 2009)

From the Athletic Department

The following position is available at:

BUCKTAIL AREA HIGH SCHOOL

  • Assistant Boys Basketball Coach – This is for the winter season of 2009-2010 which starts around November 20, 2009 and usually runs into February.

Applications for this position will be received in the Athletic office at CMHS until the position has been filled.

Applications are available in Central Mountain High School Athletic Office or Bucktail High School.

All applicants must either be a Keystone Central School District employee or have a current Act 34 Clearance, Act 151 Clearance and the new required FBI Federal Criminal History Record (fingerprint clearance). The forms and information for these clearances are available at the personnel office in the administration building or the Athletic office at CMHS.

FBI Federal Criminal History Records for Prospective Employees

All student teachers and prospective employees, including independent contractors and their employees and bus drivers, who have direct contact with children, must provide to their employer a copy of their Federal Criminal History Record that cannot be more than one (1) year old.  This only applies to employees hired on or after April 1, 2007.  Employees hired prior to April 1, 2007, are only required to provide the Federal Criminal History record if they have lived outside of the state for at least two years immediately preceding their application for employment.

The Process

The fingerprint-based background check is a multiple-step process:

  1. The applicant MUST REGISTER PRIOR to going to the fingerprint site.  Walk in service without prior registration will not be provided at any fingerprinting location.  Registration is completed online or over the phone.  Registration is available online 24 hours/day, seven days per week at www.pa.cogentid.com.  Telephonic registration is available at 1-888-439-2486 Monday through Friday, 8am to 6pm EST.   During the registration process, all demographic data for the applicant is collected (name, address, SSN, etc.) so there is no data entry required at the fingerprint collection site.

  2. The applicant will pay a fee of $36.00 (of as December, 2008) for the fingerprint service and to secure the Criminal History Record.  Applicants may make their payment online at www.pa.cogentid.com  using a credit card or debit card.  Money orders or cashiers checks payable to Cogent Systems will be accepted on site for those applicants who do not have the means to pay electronically.  No cash transactions or personal checks are allowed.

  3. The applicant proceeds to the fingerprint site of their choice (see sites below) for fingerprinting.  The location of the fingerprint sites and days and hours of operation for each site will be posted on Cogent Systems’ website at www.pa.cogentid.com.  The location of fingerprint sites may change over time so applicants are encouraged to confirm the site location nearest to their location.

  4. At the fingerprint site the Applicant Livescan Operators (ALO) manage the fingerprint collection process.

  5. The fingerprint transaction begins when the ALO reviews the applicant’s qualified State or Federal photo ID before processing the applicant’s transaction.  A list of approved ID types may be found on the Cogent Systems’ website at www.pa.cogentid.com.  Applicants will not be processed if they cannot produce an acceptable photo ID.

  6. After the identity of the applicant has been established, all ten fingers are   scanned to complete the process.  The entire fingerprint capture process should take no more than three to five minutes.

  7. The applicant’s scanned fingerprints will be electronically transmitted to the Pennsylvania State Police, who in turn submits the fingerprints and demographic information to the FBI as required by federal statute.

  8. PDE will receive the Federal Criminal History Record from the FBI.  PDE’s School Services Unit will return the Federal Criminal History Record to the applicant.  The Record will be printed on standard 8.5” X 11” paper with the Commonwealth Seal imbedded on the paper.  This document constitutes an official Record.  If an applicant presents their Federal Criminal History Record and the Commonwealth Seal is not embedded on the paper, it should be considered as invalid and not an official Record.  If the applicant does not receive the Criminal History Record from PDE within eight weeks after being fingerprinted, they should call (717) 783-3750 or email PDE at dwolfgang@state.pa.us.

  9. The applicant will then provide the Federal Criminal History Record to their prospective employer.
